We are seeking a highly skilled and dedicated Special Needs Coordinator to join our team at Distinctive Home & Health Care. As a key member of our healthcare team, you will play a vital role in supporting the unique needs of our clients and their families.
Key Responsibilities- Support healthcare professionals in managing the installation EFMP-M program, ensuring timely identification of special medical and educational needs.
- Collaborate with multi-disciplinary teams to provide effective outreach and identification of special needs, ensuring family members are appropriately "Q coded" (EFMP-enrolled).
- Ensure accurate data entry in AF-provided data management systems, providing support to healthcare professionals and training to installation personnel and medical staff.
- Integrate with the Family Member Relocation Clearance (FMRC) process, ensuring all requirements are implemented by EFMP-M staff.
- Provide support to healthcare professionals to ensure every AF sponsor with special needs has a SN file maintained at the Medical Treatment Facility (MTF).
- Collaborate with the Integrated Delivery System (IDS) and other interagency forums to provide family medical information and referrals to base and civilian agencies.
- Master's degree in Social Work (MSW) and a Bachelor's degree in Nursing.
- 2 years of experience in a clinical social work setting.
- Current, unrestricted clinical license to practice social work and a Registered Nurse license.
