Nurse Case Manager

The Nurse Case Manager (NCM) I is a vital member of the clinical team at MUSC Health Charleston, reporting directly to the Manager of Case Management and Care Transitions. This role is essential in coordinating comprehensive care plans to achieve optimal clinical outcomes and facilitate safe, timely discharges.
Key Responsibilities:- Manage the plan of care throughout the patient’s journey within the healthcare system.
- Serve as a clinical expert in discharge planning and resource utilization.
- Collaborate with healthcare professionals to ensure efficient patient movement through the facility.
This position involves working five 8-hour shifts from 8:00 AM to 4:30 PM, with the requirement of two weekend shifts per contract. Flexibility for holidays may be necessary.
Qualifications:- Experience: Minimum of 3 years of RN experience is required. Travel experience is essential, and first-time travelers will not be considered.
- Education: A Bachelor of Science in Nursing (BSN) is mandatory.
- Preferred Skills: Previous leadership experience and familiarity with Epic systems are advantageous.
The facility may call off one shift every two weeks, and all requests for time off must be submitted at the time of application. Approval will depend on the facility's operational needs. Local candidates may be considered on a case-by-case basis at a reduced rate.
Applicants must possess a valid SC or compact state nursing license at the time of employment.
