About the Role

We are seeking a skilled and compassionate Healthcare Professional to join our team as a Nurse Practitioner or Physician Assistant in our Senior Community Care (SCC) program.

Key Responsibilities
  • Conduct comprehensive assessments of patients to identify their medical needs and develop personalized care plans.
  • Manage patients with acute medical situations or exacerbations of chronic conditions, including testing, diagnostics, and interventions.
  • Provide care to patients during short-term sub-acute rehabilitation episodes.
  • Ensure accurate and complete documentation of patient information, including ICD-10 conditions.
  • Communicate and collaborate with the interdisciplinary care team to ensure seamless care coordination.
  • Conduct advanced illness and advanced care planning conversations with patients and their families.
  • Provide patients and caregivers with counseling and education on health management and disease prevention.
Requirements
  • Education: Master of Science in Nursing or Doctor of Nursing Practice (NP) or Physician Assistant degree (PA) from an accredited program.
  • Active and unrestricted license in the state where you reside, or ability to obtain by start date.
  • Active certification as a Nurse Practitioner or Physician Assistant through a national board.
  • Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating various dwelling conditions.
  • Ability to gain a collaborative practice agreement, if applicable in your state.
  • Access to reliable transportation.
Preferred Qualifications
  • 1+ years of clinical experience in a long-term care setting.
  • Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs.
  • Understanding of Geriatrics and Chronic Illness.
  • Understanding of Advanced Illness and end-of-life discussions.
  • Proficient computer skills, including the ability to document medical information with written and electronic medical records.
