RN - Intensive Care Unit Nurse

At HCA Florida Fawcett Hospital, we are seeking a dedicated and compassionate Registered Nurse to join our team as an Admission and Discharge Specialist. As a key member of our healthcare family, you will play a vital role in delivering exceptional patient-centered care.
About the Role:- Deliver seamless and efficient transitions for patients coming in and out of acute care hospital settings.
- Conduct specialized assessments to ensure patients are placed in the appropriate care areas and discharged.
- Provide education and post-hospitalization care as necessary.
- Communicate effectively with physicians and care teams to ensure proper inpatient care unit admission.
- Complete and facilitate the completion of required documentation, including admission assessments, risk assessments, medication reconciliation, discharge paperwork, and post-hospital documentation.
- Teach patients/families/caregivers about a patient's medical condition, treatment plan, medications, and possible side effects.
- Coordinate with House Supervisors/Nursing Administration to prioritize workload and identify greatest need areas to ensure smooth patient transitions.
