About the Role

Tufts Medicine Care at Home is seeking a skilled Home Health RN to join our team. As a key member of our healthcare team, you will provide comprehensive home health services to clients in their place of residence.

Responsibilities
  • Assess patients' physical and psychosocial needs in a sensitive and caring manner.
  • Develop and implement individualized plans of care in collaboration with patients, families, and healthcare providers.
  • Provide education and support to patients and families to promote health and well-being.
  • Collaborate with other healthcare professionals to coordinate care and ensure seamless transitions.
  • Manage and prioritize a caseload to ensure timely and effective delivery of services.
Requirements
  • Massachusetts RN licensure.
  • Valid state-issued driver's license.
  • One year of acute medical/surgical nursing experience.
  • CPR certification.
