Key Responsibilities

  • Conduct Thorough Assessments
    • Conduct comprehensive assessments of patient health, including medical history, physical condition, and mental status
    • Document patient information accurately and thoroughly
    • Communicate effectively with healthcare teams to ensure seamless care
  • Collaborate with Healthcare Teams
    • Work closely with physicians, nurses, and other healthcare professionals to develop and implement patient care plans
    • Participate in interdisciplinary rounds and meetings to discuss patient progress and concerns
Requirements
  • Current LPN License
    • Valid LPN license in the state of Indiana
  • Strong Clinical Skills
    • Proficient in medication management and administration
    • Ability to conduct thorough assessments and accurately document patient information
  • Excellent Communication Skills
    • Strong interpersonal and communication skills, with the ability to work effectively with patients, families, and healthcare teams
