Job Opportunity

We are seeking a skilled Nurse Practitioner to join our team at HarmonyCares, a leading provider of home-based primary care services. As a key member of our team, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

About the Role

This is a full-time position, Monday through Friday, with no weekends or holidays. You will be responsible for providing primary care to high-acuity patients in their homes, working closely with our support team to ensure seamless care delivery.

Responsibilities

  • Provide high-quality, patient-centered care to patients in their homes
  • Conduct thorough assessments, develop care plans, and implement treatment strategies
  • Collaborate with our support team to ensure timely and effective care delivery
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments

Requirements

  • Licensed as a Nurse Practitioner in the state where our office is located
  • Federal DEA license/eligibility
  • Geriatric training/experience preferred
  • Active CPR/BLS certification
