We are seeking a skilled Registered Nurse to join our Case Management team as a Care Manager RN Per Diem/Evening Specialist. This is a part-time position that requires a strong background in healthcare and excellent communication skills.
Key Responsibilities- Collaborate with patients, caregivers, and healthcare teams to develop and implement effective care plans.
- Conduct thorough assessments and evaluations to identify patient needs and develop strategies for optimal care.
- Communicate with payers, physicians, and other healthcare professionals to ensure seamless care coordination.
- Provide education and support to patients and families to promote self-management and independence.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Bachelor's Degree in Nursing or equivalent experience.
- California Registered Nurse License upon hire.
- 2 years of experience in healthcare, with a focus on case management or care coordination.
- Strong communication and interpersonal skills, with the ability to work effectively in a team environment.
- Bachelor's Degree in Nursing.
- Master's Degree in Healthcare related field.
- Certification in Case Management (ACM or CCM) upon hire.
