POSITION OVERVIEW


The MDS Coordinator is responsible for overseeing and facilitating the patient assessment process in compliance with state regulations and the strategic objectives of Hamilton Medical Center.

KEY RESPONSIBILITIES

  • Ensures effective communication among nursing center departments and between patients and their families to enhance the assessment and care planning process.
  • Maintains clear communication with consultants and interdisciplinary team members.
  • Utilizes strong interviewing techniques to gather comprehensive information from patients, families, and staff.
  • Conducts and organizes patient or family interviews for assessment purposes.
  • Evaluates patient conditions and medical histories to identify special needs or necessary adjustments to initial assessments.
  • Possesses an in-depth understanding of nursing resources and integrates relevant information into care plans.
  • Coordinates the scheduling of assessment and care plan meetings, ensuring appropriate participation from all relevant parties.
  • Develops care plans that accurately reflect patient needs, interventions, and measurable outcomes.
  • Ensures timely completion of patient assessments and care plans in accordance with current regulations.
  • Facilitates educational sessions to inform staff about patient care plans prior to care delivery.
  • Reviews assessments and care plans for accuracy, ensuring all necessary signatures and dates are included.
  • Monitors the MDS process for compliance with established guidelines and standards.
  • Provides training to the interdisciplinary team and staff regarding the MDS process.
  • Conducts physical assessments of patients as required for the MDS process.
  • Assesses patient needs and recommends appropriate rehabilitative and maintenance therapy services.
  • Organizes patient care conferences to discuss care strategies.
  • May lead educational programs related to patient assessment and care.
  • Collaborates with all shifts to plan and implement care effectively.
  • Assists in ensuring compliance with federal, state, and local regulations regarding patient care plans.
  • May support management with reports and special projects as needed.
  • Adheres to specific timelines for scheduling and planning patient care.

ADMINISTRATIVE DUTIES

  • Participates in nursing center surveys and prepares necessary reports.
  • Engages in continuing education to stay updated on industry changes and maintain licensure.
  • Attends mandatory training sessions.
  • Upholds patient rights to fair treatment, privacy, and individuality.
  • Complies with corporate compliance protocols.
  • Reports any job-related hazards, including exposure to bodily fluids.
  • Follows established safety procedures and regulations.
  • Performs additional duties as assigned by supervisors.
  • Maintains confidentiality and adheres to privacy and security policies.
  • Communicates effectively with patients and families, providing a welcoming and supportive environment.

QUALIFICATIONS

Must possess a nursing degree from an accredited institution. A valid RN nursing license is required, along with a minimum of one year of supervisory experience in a healthcare setting.

  • Frequent standing, walking, and sitting throughout the workday.
  • Proficient use of medical equipment and technology.
