The Licensed Vocational Nurse (LVN) - Wellness Coach is essential in managing care for a specific group of clients, ensuring the effective execution and supervision of their treatment plans. Key Responsibilities:

- Actively engage in the treatment team, providing critical insights.
- Ensure timely completion of all required documentation and charting in electronic health records as per program standards.
- Oversee medication orders and refills in partnership with pharmacies.
- Address client inquiries regarding medications promptly.
- Conduct comprehensive assessments of clients' psychiatric history and physical health needs.
- Educate clients about medications and their potential side effects.
- Collaborate with Psychiatrists and Registered Nurses to monitor client wellness, medication adherence, effectiveness, and side effects.
- Respond swiftly to medical emergencies and document vital signs accurately.
- Initiate timely interventions and provide crisis support as necessary.
Qualifications:
- Completion of a Vocational Nursing diploma from an accredited institution.
- Current licensure as a Vocational Nurse in California.
- A minimum of three years of experience in treating psychiatric patients.
- Bilingual proficiency in Spanish is preferred for roles serving Spanish-speaking clients.
- Experience with psychiatric and dually diagnosed patients is advantageous.
- Proficiency in Microsoft Office Suite and Electronic Health Record (EHR) systems.
- Strong problem-solving abilities and capacity to manage emergency situations effectively.
- Able to work independently while also collaborating effectively within a team.
