About the Role

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at AcuteCare Health System LLC. As a LPN, you will play a vital role in providing high-quality care to our elderly patients.

Key Responsibilities

  • Administer medications and implement primary care orders
  • Evaluate participant complaints physically and/or by telephone
  • Provide education to participants and caregivers on care plans, nursing treatments, and medications
  • Communicate participant conditions to primary caregivers, physicians, and multidisciplinary teams

Requirements

  • Graduate of an accredited school of nursing
  • Current LPN license in good standing to practice in the required state
  • IV Certification preferred
  • Experience with paperless documentation preferred
  • 1 year of experience working with a frail or elderly population preferred
  • Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance
  • Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact
  • Pass a comprehensive criminal background check
