Key Responsibilities:

Nutrition Consultations: Nutrition consultations are addressed within one business day of the request, with documentation completed in the electronic medical record (EMR).

Nutrition Assessments: Conduct assessments for patients at San Juan Regional Medical Center and San Juan Regional Rehabilitation Hospital identified as at nutritional risk, based on screening tools, within two days, and document findings in the EMR.



Follow-Up Evaluations: The care plan for patients at nutritional risk is reviewed and documented within one week of the initial assessment and adjusted as necessary.

Nutrition Education:
 Provide nutritional education to both inpatients and outpatients at San Juan Regional Medical Center, San Juan Regional Rehabilitation Hospital, and the Cancer Center to empower self-management of medical nutrition therapy.



Nutrition Workshops:
 Design and conduct workshops (e.g., cardiac health, diabetes management, pulmonary health, weight control) with up-to-date nutritional information for patients, healthcare providers, and nursing students. Ensure education is delivered at an understandable level for patients and caregivers, with documentation completed in the EMR within 24 hours.



Outpatient Medical Nutrition Therapy:
 Deliver educational sessions to outpatients, ensuring information is accessible. Provide documentation of individual medical nutrition therapy to the provider's office within one week.



Community Nutrition Initiatives:
 Develop and present relevant nutrition topics based on community needs. Collaborate with marketing when appropriate and complete community reporting as required.



Committee Participation:
 Actively participate in committee meetings, report progress to the clinical team, and serve as an educational resource for staff.



Clinical and IT Collaboration:
 Engage in projects that intersect clinical practice and information technology; assist with staff training as needed.



Telephone Inquiries:
 Respond promptly to phone inquiries, aiming to return calls within 24 hours if a message is left.

CDR Registration & State Licensure:
Maintain professional growth and development to ensure current knowledge, CDR registration, and state licensure.



Departmental Education:
 Attend departmental in-services and meetings, and participate in quality assurance and performance improvement programs as assigned.



HealthStream Compliance:
 Complete all annual HealthStream modules with 100% compliance.
