  • This role involves collaboration with the Palliative Care Medical Director and other palliative team members to deliver palliative care by providing diagnosis, treatment, and follow-up for patients referred to palliative care.
  • You will be responsible for patient palliative care from the initial assessment through discharge.

  1. Developing and implementing individualized care plans for patients, in consultation with the patient, family, and other members of the Palliative Care Team.
  2. Providing education and support to patients and their families to enhance their understanding of palliative care.
  3. Collaborating with other healthcare providers to ensure seamless care coordination.

  • Strong clinical skills and knowledge of palliative care principles.
  • Excellent communication and interpersonal skills.
  • A collaborative agreement in place with a supervising physician agreeable to supervise work in palliative care program.
