The Palliative Care Nurse Practitioner will play a vital role in delivering high-quality, compassionate care to patients with serious illnesses.
">- This role involves collaboration with the Palliative Care Medical Director and other palliative team members to deliver palliative care by providing diagnosis, treatment, and follow-up for patients referred to palliative care.
- You will be responsible for patient palliative care from the initial assessment through discharge.
Your key responsibilities will include:
">- Developing and implementing individualized care plans for patients, in consultation with the patient, family, and other members of the Palliative Care Team.
- Providing education and support to patients and their families to enhance their understanding of palliative care.
- Collaborating with other healthcare providers to ensure seamless care coordination.
To succeed in this role, you will require:
">- Strong clinical skills and knowledge of palliative care principles.
- Excellent communication and interpersonal skills.
- A collaborative agreement in place with a supervising physician agreeable to supervise work in palliative care program.
