As a key member of our team, you will be responsible for delivering personalized care to patients, promoting their health and well-being, and collaborating with our multidisciplinary team to ensure seamless care coordination.
Responsibilities- Provide one-to-one patient care in a home environment, tailoring your approach to meet the unique needs of each patient.
- Execute physicians' prescribed plans of care, documenting all care provided in our electronic system of record.
- Administer prescribed medication, treatments, and therapies, ensuring patient safety and comfort.
- Conduct patient assessments, coordinate care, and provide health education to patients and their families.
- Current licensure as a Licensed Practical Nurse (LPN) in good standing.
- At least 6 months of hands-on nursing experience.
- Current CPR certification.
- Ability to provide in-home nursing care to patients of all ages.
