About This Role

We are seeking a highly skilled and experienced Care Management Supervisor to join our team at MedStar Health. As a key member of our Care Transformation Organization, you will play a critical role in ensuring the delivery of high-quality care management services to our patients.

Key Responsibilities
  • Leadership and Supervision: Provide leadership, instruction, supervision, and direction to a team of care managers and care coordinators. Assist with the selection, training, and orientation of new staff members.
  • Care Management: Develop and implement individualized plans of care for patients, ensuring timely access to medical, psychiatric, psychosocial, and other essential services. Act as an advocate for patients and their families, facilitating communication and collaboration among healthcare providers and community resources.
  • Operations and Productivity: Manage day-to-day operations of the care coordination team, including preparation of daily coverage assignments, staffing calendars, and productivity reports. Analyze reports and identify opportunities for process improvements.
  • Communication and Collaboration: Communicate effectively with patients, families, healthcare providers, and community resources to ensure seamless care coordination. Facilitate ongoing communication and collaboration among practice staff and care team members.
  • Quality and Service Improvement: Participate in multidisciplinary quality and service improvement teams to identify opportunities for process improvements and implement changes to enhance patient care and satisfaction.
Requirements
  • Education: Associate's degree in Nursing; BSN preferred.
  • Experience: 5-7 years of diverse clinical utilization management experience, with 3 years of case management experience required; leadership experience preferred.
  • Licenses and Certifications: Valid RN license in Maryland; Certified Case Manager (CCM) certification preferred.
  • Knowledge, Skills, and Abilities: Ability to use computer to enter and retrieve data; working knowledge of Microsoft Word, Excel, and PowerPoint applications; effective verbal and written communication skills; ability to run and analyze departmental productivity reports.
