Job Title:

Person-Centered Care Coordinator

Description:

In this role, you will work closely with participants and their families to develop and implement personalized plans that promote independence, dignity, and inclusion. You will provide direct support and advocate for participants to ensure they receive the services and support they need.

Responsibilities:
  • Develop and maintain relationships with participants and their families.
  • Conduct assessments and develop personalized plans that promote independence and inclusion.
  • Coordinate with community resources and services to ensure participants receive the support they need.
  • Advocate for participants to ensure they receive fair and respectful treatment.
Requirements:

QUALIFICATIONS:

  • MUST be at least 21 years of age.
  • MUST pass a criminal background check.
  • MUST have a current California driver's license with a good driving record.
  • Can lift a minimum of 45lbs.
  • Strong communication, teambuilding and leadership skills.
  • Willingness to learn and be flexible.
  • Excellent interpersonal, negotiation, and conflict resolution skills.
  • Excellent organizational skills and attention to detail.
  • Excellent time management skills with a proven ability to meet deadlines.
Salary and Benefits:

We offer a competitive salary range of $45,000 - $55,000 per year, depending on experience, as well as health, vision, and dental benefits, and a 403B plan with employer match.
