Nurse Care Coordinator

Job Summary
The On-Call Registered Nurse is responsible for organizing and directing hospice care, providing comprehensive assessments, and developing individualized care plans. This role requires excellent communication and problem-solving skills, as well as the ability to work independently and as part of a team.
Key Responsibilities
- Conduct initial and ongoing comprehensive assessments of patients and families to determine hospice needs.
- Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and families.
- Provide professional nursing care, including administering medications and treatments as prescribed by physicians.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Document patient information and care plans in a timely and accurate manner.
Requirements
- Graduate of an accredited school of nursing with current licensure in the state.
- One to two years of recent acute care experience in an institutional setting and one to two years of recent experience in home care.
- Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
- Prolonged or considerable walking or standing, able to lift, position, and/or transfer patients, and able to lift supplies and equipment.
