Job Summary

We are seeking an Independent Optometrist to lease space in our Walmart stores and provide high-quality eye and vision care services to our customers.

Key Responsibilities
  • Provide comprehensive eye exams and vision care services to patients
  • Establish and manage your own patient schedule and fee structure
  • Have autonomy to control your days of practice and hours of operation
  • Have access to equipment and instrumentation for a fair market value
  • Option to opt into administrative and/or Opt-Tech support services (if state allows)
Requirements
  • Doctor of Optometry (OD) from an accredited optometry school
  • Valid state optometry license
  • Ophthalmologist (MD or DO) from an accredited school
  • Valid state ophthalmology license
  • General Liability Certificate
  • Professional Liability Insurance Certificate
  • W-9
