About the Position

Plymouth Housing Group seeks a highly motivated and compassionate Housing Case Manager to join our team. In this role, you will work closely with residents to address their housing stability needs, provide support and resources, and facilitate connections with community agencies and services.

The estimated salary for this position is $68,700 per year, based on a 40-hour workweek.

Key Responsibilities
  • Housing Service Planning: Develop and maintain housing service plans that outline goals and strategies for resident stability, addressing transition needs and long-term housing stability requirements.
  • Care Coordination: Collaborate with residents to schedule appointments, arrange transportation, and accompany them to appointments as needed, ensuring continuity of care.
  • Community Building: Foster a sense of community among residents, staff, and community partners, promoting teamwork and collaboration.
Qualifications
  • Education: Bachelor's degree in a human service field or equivalent experience.
  • Experience: Minimum 2 years of experience working with homeless and low-income individuals, including those with mental and physical illnesses, chemical dependency/addictions, disabilities, and HIV/AIDS.
  • Licensure/Certification: Valid Washington State Driver License and insurable driving record.
  • Skills and Abilities: Excellent written and verbal communication skills, solid organizational and time management skills, self-motivation, creative problem-solving, ability to develop and sustain relationships, empathy, patience, flexibility, and leadership experience.
