Responsibilities
Your key responsibilities will include:
* Providing comprehensive primary care services to veterans in their homes
* Conducting assessments, developing care plans, and implementing interventions to address physical, emotional, and social needs
* Collaborating with interdisciplinary teams to ensure seamless care coordination
* Educating patients and caregivers on self-management techniques, disease prevention, and health promotion
Requirements
To be successful in this role, you'll need:
* A current, active RN license in the United States or its territories
* At least 1 year of experience as a registered nurse, preferably in home health or primary care settings
* Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations
* Strong clinical judgment, problem-solving, and critical thinking skills
* Familiarity with electronic medical records and practice management systems
* Ability to travel up to 50% of the time, depending on business needs
