Sparrow Health System is seeking a skilled Healthcare Liaison Specialist to join our team. As a key member of our population health management team, you will play a vital role in coordinating care for patients with complex needs.
Job Summary:
We are looking for a highly motivated and organized individual to work as a liaison between our healthcare teams and patients. Your primary responsibility will be to provide exceptional patient care and coordination services, ensuring seamless transitions between healthcare settings.
Main Responsibilities:
* Provide patient education and support on self-management techniques and concepts.
* Collaborate with healthcare teams to develop and implement comprehensive care plans.
* Coordinate with payers to secure necessary benefits and services for patients.
* Facilitate communication between patients, families, and healthcare providers to ensure continuity of care.
Requirements:
* Registered Nurse with a current Michigan license.
* National case management certification preferred.
* Minimum 3 years acute care clinical experience as a registered nurse.
* Bachelor's Degree in Nursing or healthcare-related field OR complete within 2 years of hire.
