About the Role

We are seeking a highly skilled and dedicated MDS Coordinator to join our team at Sodus Rehab & Nursing Center. As a key member of our care team, you will play a vital role in ensuring the highest quality of care for our residents.

Key Responsibilities
  1. Conduct thorough assessments of residents to identify their care needs and develop personalized care plans.
  2. Monitor and coordinate the completion of Minimum Data Set (MDS) assessments and care plans to ensure timely and accurate documentation.
  3. Collaborate with interdisciplinary teams to ensure that care plans are aligned with resident needs and goals.
  4. Develop and maintain accurate and up-to-date care plans, ensuring that they reflect current resident needs and changes.
  5. Participate in care conferences and contribute to the development of care plans that promote resident well-being and quality of life.
  6. Monitor and report on resident incidents and accidents, ensuring that care plans are updated to reflect changing resident needs.
  7. Stay up-to-date with current regulations and guidelines, ensuring that care plans and documentation are compliant.
  8. Collaborate with the Director of Nursing Care to address persistent issues related to non-compliant documentation.
  9. Participate in ongoing education and training to enhance knowledge of current regulations and best practices.
Requirements
  • Graduate of an accredited school of nursing in preparation for Registered Nurse or Licensed Practical Nurse licensure.
  • One year previous experience in a long-term care environment, with knowledge of MDS and care plan processes.
  • Basic knowledge of PPS and RUGs.
  • Must be a Registered Nurse or Licensed Practical Nurse licensed by the Virginia Board of Nursing.
  • Excellent verbal and written communication skills, with the ability to work effectively with interdisciplinary teams.
