Job Summary

Nascentia Health is seeking a skilled Clinical Educator to join our team. As a Clinical Educator, you will play a vital role in the initial and ongoing education process for clinical staff within our Certified Home Health Agency (CHHA) and Licensed Home Care Services Agency (LHCSA).

Key Responsibilities
  • Develop and implement education programs to enhance clinical staff skills and knowledge.
  • Provide direct care as part of maintaining individual skills and for the purpose of mentoring others.
  • Ensure quality and safe delivery of home health services.
  • Instruct new staff in agency policy and procedure.
  • Verify appropriate technique in demonstration of new skills in both the home and simulated setting.
  • Provide support and ongoing education for existing staff with identified performance deficits.
  • Provide individualized guidance to clinical staff through joint visits, direct observation, chart review, and other methods as needed.
  • Ensure compliance of new staff or existing staff with established standards of documentation to provide evidence of skilled, medically necessary intervention.
  • Employ the principles of adult learning to convey new or changed information or to elicit performance improvement with staff.
  • Mentor new staff in the role and responsibilities of a case manager.
Requirements
  • Current New York State Registered Nurse professional license.
  • Degree from an accredited school of nursing or college.
  • One-year experience as a public health nurse in a home care agency.
  • Current and valid driver's license and auto insurance and a vehicle which can be used for work.
  • Self-directed and organized with the ability to work independently.
  • Good oral and written communication skills.
Physical Requirements
  • Speaking/visual/hearing ability sufficient to comprehend and express written/verbal communication.
  • Ability to perform tasks involving physical activity including pushing/pulling, lifting and carrying, patient transfers, extensive bending, standing, kneeling, walking in various weather conditions, and going up and down stairs.
  • Ability to drive on a daily basis in all weather conditions.
