Job SummaryWe are looking for a dedicated and compassionate Physical Therapist to provide high-quality care to our patients in the comfort of their own homes. As a Physical Therapist with LHC Group, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.
Responsibilities- Evaluate patients' functional status and physical therapy needs, and assist physicians in developing therapy plans of care.
- Provide physical therapy services within the scope of practice, in accordance with state laws and in coordination with other healthcare team members.
- Observe and report patients' responses to treatment and any changes in their condition to supervising nurses and physicians.
- Treat patients to relieve pain, develop and restore function, and maintain maximum functioning using acceptable standards of practice.
- Instruct and inform participating healthcare team members, patients, and family/caregivers regarding plans of care, functional limitations, and progress toward goals.
- Visit patients according to plans of treatment, complete progress notes for each visit, and submit progress notes per LHC Group policy.
- Complete patient evaluations and develop PT plans of care within state-specific guidelines.
- Report outcomes of evaluations, goals, and anticipated projected frequency of care.
- Current Physical Therapy licensure in the state of Washington.
- Current CPR certification.
- Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
