Job Summary

Pinnacle Home Care Inc. is seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Care Specialist. As a key member of our care team, you will be responsible for providing high-quality patient care and support to individuals in their homes.

Key Responsibilities
  • Conduct thorough assessments of patients to determine their care needs and develop personalized care plans.
  • Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
  • Monitor patient progress, make adjustments to care plans as needed, and provide education and support to patients and families.
  • Supervise and train home health aides to ensure high-quality care delivery.
  • Participate in on-call duties and respond to patient needs outside of regular working hours.
Requirements
  • Current licensure as a Registered Nurse in the state of practice.
  • One to two years of recent acute care experience in an institutional setting, with a preference for home health care experience.
  • Excellent observation, verbal and written communication skills, problem-solving skills, and basic math skills.
  • CPR certification and ability to lift, position, or transfer patients as needed.
  • Able to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
