Job Summary

We are seeking a skilled and compassionate Nurse Navigator to join our team at Methodist Hospitals. As a Nurse Navigator, you will play a vital role in promoting patient-centered care and ensuring that our patients receive the highest quality of care.

Key Responsibilities
  • Assess patient needs and develop individualized care plans to meet those needs
  • Collaborate with healthcare teams to ensure seamless care coordination and communication
  • Provide education and support to patients and their families to promote health and wellness
  • Monitor patient progress and adjust care plans as needed
  • Participate in quality improvement initiatives to enhance patient care and outcomes
Requirements
  • Current licensure as a Registered Nurse in the state of Illinois
  • 2+ years of experience in a healthcare setting, preferably in a navigator or coordinator role
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
  • Strong analytical and problem-solving skills
