Job Summary

The successful candidate will provide clinical pharmacy management for home infusion therapy patients, performing initial and ongoing patient assessments, interpreting prescriptions and physicians' orders, and ensuring the safe and effective delivery of medication.

Key Responsibilities
  • Provide clinical pharmacy management for home infusion therapy patients, including initial and ongoing patient assessments.
  • Interpret prescriptions and physicians' orders, and perform computer entry.
  • Ensure the safe and effective delivery of medication, including intravenous admixture and product distribution functions.
  • Discuss patient care with other healthcare professionals, including physicians and home health nurses.
  • Identify potential adverse drug events and review reported adverse events to identify opportunities for improvement.
  • Rotate 24/7 on-call coverage with other pharmacists.
  • Demonstrate knowledge and skills necessary to provide patient care appropriate to the ages of the patients served.
Requirements
  • Minimum of a Bachelor of Science Degree in Pharmacy.
  • Active Illinois pharmacist license.
  • Maintains a valid driver's license.
  • Provides own transportation and auto insurance and abides by all laws, rules, and recommendations for safe driving while on duty.
  • Knowledgeable of state, federal, and JCAHO regulations governing the provision of services.
  • Analytic skills necessary to organize and oversee the work of others and evaluate quality and outcome of care provided.
  • Ability to work independently and execute own workload.
  • Ability to use computer workstation, operate infusion pumps, clean room pharmacy equipment, and office equipment.
  • Interpersonal skills for effective verbal and written communication.
