Job Summary:Our ideal candidate will have a strong background in physical therapy, with experience in home health settings. You will be responsible for planning, organizing, and conducting physical therapy treatment based on medical referrals and evaluations. Your excellent interpersonal skills and ability to communicate effectively will enable you to work closely with clients, their families, and other healthcare professionals to ensure seamless care.
Responsibilities:- Perform clinical assessments and identify patient needs and appropriateness of client care.
- Develop and implement Plan of Care (POC), progress notes, phone orders, and other clinical record documentation.
- Apply concepts of infection control and universal precaution in coordination/performing client care activities.
- Provide appropriate treatment for neuro-muscular, neurological, and orthopedic disorders based on medical referral and therapy assessment.
- Assist/instruct client/family in transfer techniques and instruct on use of equipment.
- Provide effective communication to clients, their families, team members, and other healthcare professionals.
- Monitor assigned cases to ensure compliance with requirements of third-party payers.
- Complete appropriate documentation in a timely manner to assure compliance with company policy.
- Two (2) years of experience as a PT in a clinical care setting.
- Home health experience.
- Therapy skills as defined by generally accepted standards of practice.
- Excellent interpersonal skills and ability to communicate effectively.
- Demonstrates proven decision-making skills.
- Current driver's license in good standing.
- Reliable transportation and auto liability insurance.
- Current State professional license as a PT in any/all applicable states.
