Job Description: We are seeking a Registered Nurse to join our team as a Clinical Care Coordinator in our hospital unit. The successful candidate will provide exceptional patient care, collaborate with the healthcare team, and ensure seamless transitions throughout the patient's journey. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Responsibilities:

  • Collaborate with the patient and/or family, physician, and other members of the interdisciplinary healthcare team to develop a plan of care
  • Ensure continuity of care and smooth transition post-discharge from one level of care to another
  • Provide direct care to assigned patients consistent with the established nursing plan
  • Collects data and completes nursing assessments utilizing appropriate area-specific standards/policies and tools
  • Develops an individual plan of care based on the age and assessed needs of the patient

Requirements:

  • Associate's Degree in Nursing
  • Required licensure/registrations/certifications: registered nurse license in South Carolina
  • In lieu of 1 year of registered nurse experience, will accept successful completion of the Prisma Health Nurse Residency Program or successful completion of a comparable residency from another organization

Salary: $80,000 - $110,000 per year
