Nurse Practitioner

Horizon Health is seeking a dedicated and compassionate Registered Nurse, Licensed Practical Nurse, or Certified Medical Assistant to join our team in Bryant, SD. As a valued member of our outpatient clinic, you will provide high-quality patient-centered care and contribute to the health and wellness of our communities.
Key Responsibilities- Coordinate patient care through entire visit, including rooming, vitals, history, and medication updates.
- Provide patient education on health management, referral management, and care coordination.
- Initiate and perform nursing skills, including injections, vaccinations, labs, and x-rays.
