Title:
Physical Therapist - Home Health Specialist

Role:
As a Home Health Physical Therapist with HexaQuEST Health, Inc., you will be responsible for providing high-quality physical therapy services to patients in their homes. This role requires strong clinical skills, excellent communication, and the ability to work independently.

Responsibilities:

  • Prioritize start of care visits to ensure timely delivery of services.
  • Supervise Physical Therapy Assistants to ensure quality care and efficient operations.
  • Report start of care visits the next day and submit clinical notes and documentation within 48 hours of each visit.
  • Maintain open communication with supervisors, intake, and office staff to ensure seamless operations.
  • Respond promptly to calls and messages during regular business hours.

Requirements:
A current California Physical Therapy License and at least one year of experience in Home Health are required for this position.

Territory:
This role is based in Sacramento, CA, with the possibility of expanding to other areas as needed.

Schedule:
The schedule for this position is 40 hours per week, Monday through Friday, with one weekend rotation per month.

Training:
New hires will receive three days of training, including one day in the office and two days in the field.

Equipment:
You will be issued an iPad and a Telecommunication One Talk device for use in the field.

Mileage Reimbursement:
You will be reimbursed for mileage at a rate of *** per mile, from the first to the last patient.

Parking and Toll Reimbursement:
Parking and toll expenses will be reimbursed.

Company Car:
You may be eligible for a company car with a reduced bill rate.

Bill Rate:
The bill rate for this position is *** OT Rate: +*** paid after 30 points per week.

Holiday Rate:

On-Call Rate:

Call Back Rate:
N/A.

Productivity Requirement:
You are expected to achieve 30 points per week, with a point-to-hourly conversion rate of 1 = 1.33 hours. This translates to an average of 8 hours per day and 40 hours per week. Overtime must be pre-approved by the Clinical Supervisor.
