Hawai'i Island Community Health Center seeks a skilled Clinical Pharmacist to join our team in providing high-quality patient care.

Job Summary

We are looking for a detail-oriented and communicative pharmacist with excellent medication management skills to work collaboratively with healthcare providers and support staff.

Key Responsibilities
  • Provide comprehensive medication therapy services, including assessments, evaluations, and recommendations for patients' medications.
  • Collaborate with healthcare providers to develop and implement personalized medication plans that align with patients' goals of care.
  • Supervise student pharmacists engaged in Advanced Pharmacy Practice Experiences (APPE) and/or Introductory Pharmacy Practice Experiences (IPPE).
  • Participate in policy and procedure development, ensuring adherence to organizational, regional, national, state, and federal requirements and guidelines.
Requirements

To be successful in this role, you will need:

  • A BS Pharmacy Degree or a Doctor Pharmacy Degree from an accredited College/School of Pharmacy.
  • Licensed or eligible for licensure as a pharmacist in the State of Hawaiʻi.
  • Completion of American Society of Health Systems Pharmacists (ASHP) accredited PGY-1 pharmacy residency is preferred.
  • Board Certification in either Ambulatory Care or Pharmacotherapy is preferred.

In exchange for your expertise, we offer a competitive salary range of $$130,000 - $160,000 per year, depending on experience. This estimate is based on the current market rate for a Clinical Pharmacist in Hawai'i.
