Multilingual Caseworker

Trustbridge, a community nonprofit, provides support for families facing serious illness 24 hours a day. This role is part of a team that has merged three hospices into one branch.

As a Bilingual Caseworker, you will be responsible for assessment and intervention with patients and families within your scope of practice as a member of the interdisciplinary team.

The position involves assessment, intervention, care planning, outcome evaluation, documentation, and collaboration/communication with the Interdisciplinary Team to assist patients and families with their goals and assist the team in delivering hospice care.

Key Responsibilities:

• Assessment and intervention with patients and families

Requirements:

• Bachelor's degree from an approved college or university accredited by the Council on Social Work Education
• Bilingual (English/Spanish) proficiency preferred
