Job Overview

We are seeking a friendly and compassionate Primary Care Provider to join our team. As a Primary Care Provider, you will play a crucial role in providing comprehensive care to patients in long-term care facilities. Responsibilities

  • Provide comprehensive healthcare services and coordinate patient care

  • Conduct thorough patient assessments, including medical history reviews and physical examinations
  • Provide counseling to residents and their families

  • Make appropriate adjustments to care plans
  • Commitment to person-centered care
  • Create a safe and nurturing environment for elderly residents in the nursing home.
