About the Role

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Encore Communities. As a LPN, you will play a vital role in providing high-quality patient care and contributing to the overall success of our organization.

Responsibilities
  • Complete and scan required record keeping forms/charts upon the guest's admission, transfer, and/or discharge.
  • Chart nurses' notes in an informative and descriptive manner that reflects the care provided to the guest, as well as the guest's response to the care.
  • Report all discrepancies noted concerning physician's orders, diet change, charting error, etc., to the Director of Nursing.
  • Ensure that appropriate documentation concerning unauthorized discharges is entered in the guest's medical record in accordance with established procedures.
Requirements
  • Current, unencumbered LPN/LVN license required or Compact State license required.
  • A minimum of 2 years LPN/LVN experience required.
  • Able to educate patients and family on multiple disease processes and have good medication knowledge and wound care experience.
