The Care Coordinator role at Help at Home is essential for ensuring that our clients receive the highest quality of care in their own residences. This position is based in Michigan and offers a competitive starting pay between $17 and $20 per hour, with the convenience of weekly pay.
Key Responsibilities:
As a Care Coordinator, you will be responsible for:
- Conducting visits to new clients to assess their individual needs and home conditions.
- Developing comprehensive initial Care Plans tailored to each client's requirements.
- Completing all necessary documentation, including contracts and mandated disclosures.
- Interviewing potential caregivers and guiding them through the employment process.
- Maintaining a disciplined schedule for client follow-ups, performing reassessments every 120 days.
- Monitoring caregiver performance and providing necessary education and coaching.
- Addressing client complaints and documenting significant issues for management review.
Travel Requirements:
This position involves significant travel within the assigned territory, with approximately 90% of your time spent visiting clients.
Qualifications:
To be successful in this role, candidates should possess:
- Preferred certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA).
- A minimum of one year of experience in healthcare, particularly in home health settings.
- Basic to moderate computer skills, with proficiency in MS Office.
- Excellent communication and organizational abilities.
- A valid driver's license and reliable transportation.
