Primary Care Physician Assistant

We are seeking an experienced Primary Care Physician Assistant to join our team at HarmonyCares. As a Primary Care Physician Assistant, you will provide high-quality primary care services to patients in their homes, Monday through Friday.
About the RoleThe ideal candidate will have directly related experience in home health, geriatrics, hospice and primary care. You should be able to practice medicine with a focus on patient care, not volume, and take the time needed to truly address each patient's needs.
Responsibilities- Provide primary care services to high acuity patients in their homes
- Collaborate with our healthcare team to develop personalized care plans for each patient
- Maintain accurate and up-to-date patient records
- Competitive base salary: $120,000 - $150,000 per year
