Compassionate Hospice Care Specialist

About the Role:
We are seeking an experienced RN with a passion for delivering compassionate care to join our team. The ideal candidate will have a strong background in hospice nursing, excellent communication skills, and a commitment to providing personalized care to each patient.
Responsibilities:
- Provide direct patient care services to hospice patients and supportive care to the patient's family
- Act as primary liaison between physician, patient/family, and hospice team
- Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient
- Manage and collaborate with interdisciplinary teams to provide comfort, peace, and dignity
Requirements:
- RN license in the state of requested employment (or eligibility to obtain)
- 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred
- Current driver's license and automobile insurance
- Current CPR certification
