Job Summary

We are seeking an experienced Registered Nurse to join our team in providing high-quality care to patients and their families. As an After Hours RN, you will be responsible for coordinating patient care, conducting nursing assessments, and developing plans of care to meet the unique needs of each patient.

Responsibilities
  • Assume responsibility for the effective coordination of patient/caregiver care.
  • Conducts nursing assessments to assist in the overall management of patient/caregiver needs.
  • Make visits to provide appropriate interventions.
  • Follows established plan of care, evaluates patient/caregiver responses to determine progress toward goals, and revises plan of care as necessary.
  • Develops, implements, and evaluates plans of care for patients/caregivers.
  • Provides interventions consistent with the plan of care and performs all interventions required for each visit.
  • Identifies new problems or needs when they occur.
  • Facilitates patient and caregiver independence to extent possible.
  • Initiates and maintains a liaison relationship with healthcare providers to effectively coordinate multidisciplinary care.
Requirements
  • A Diploma or Associates Degree in nursing is required; Bachelor's Degree preferred.
  • A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.
  • Current CPR required; CHPN preferred.
  • Minimum 2 years related experience. Experience in direct care of patients with life-limiting illnesses required.
