Responsibilities:- Assess and Plan Care: Conduct thorough assessments of patients' physiologic health status, including gastrointestinal, cardiovascular, respiratory, renal, and neurological systems. Develop individualized plans of care to address patients' needs and goals.
- Implement and Evaluate Care: Implement care plans, coordinate with other healthcare team members, and evaluate the effectiveness of care provided. Document patient outcomes and make adjustments to care plans as needed.
- Communicate Effectively: Communicate with patients, families, and healthcare team members to ensure seamless care coordination. Provide emotional support and education to patients and families.
- Administer Medications: Administer medications safely and correctly, including IV drips and local solutions. Educate patients and families about medication use and potential side effects.
- Participate in Patient Education: Provide education to patients and families about their conditions, treatments, and self-care. Recognize and utilize health teaching opportunities to promote patient empowerment.
- Perform Related Duties: Perform other duties as required, including participating in quality improvement initiatives and collaborating with other healthcare team members.
- Graduate from an accredited School of Nursing with an Associate's Degree in Nursing (required) or Bachelor's Degree in Nursing (preferred).
- Minimum 1-2 years of professional nursing experience in the related specialty.
- Current license to practice as a Registered Professional Nurse in New York State.
- BLS/ACLS certification, plus specialized certifications as needed.
