About the Opportunity

Join our dynamic team as a Resident Health Services Director and take on a rewarding role that makes a difference in the lives of our residents.

Key Responsibilities

  • Care Coordination: Evaluate residents' needs, assist with navigating available services, and communicate with medical personnel.
  • Return Coordination: Coordinate residents' return, including transportation, medications, and meals, and update their plan of care.
  • Education and Training: Provide individual and group educational opportunities for residents and staff on disease prevention and management.
  • Service Development: Facilitate collaboration with other leaders to identify and recommend additional services or physical moves within the continuum of care.
  • Support and Guidance: Anticipate and coordinate with residents and family members when significant changes occur, necessitating additional support services or a physical transfer.
  • Emotional Support: Assist residents and family members in dealing emotionally with the aging process and illnesses.
  • Staff Oversight: Provide day-to-day oversight of medication technicians and LVN staff to ensure standards are met for delivering services to community residents.

Requirements

  • Licensure: Current California Licensed Vocational Nurse License in good standing with the LVN Board of Nursing.
  • RCFE Certification: Preferred but not required.
  • CPR, First Aid, and AED Certification: Required.
  • Experience: Five years' experience working with seniors in long-term care, home health, or other healthcare settings preferred.
  • Management Experience: Five years of organizational or management experience, supervising others preferred.
