Job Summary:

We are looking for a skilled Clinical Consultant Pharmacist to provide clinical and professional expertise on all aspects of pharmaceutical services. The successful candidate will have a strong background in medication management and a passion for collaborating with interdisciplinary teams to deliver comprehensive care.

Responsibilities:

  • Provide clinical and professional expertise on all aspects of pharmaceutical services
  • Perform monthly medication regimen and admission reviews
  • Effectively collaborate with the interdisciplinary team to provide pharmaceutical care
  • Lead the monthly Pharmacy & Therapeutics clinical team meeting
  • Challenge and grow your expertise beyond traditional regulatory requirements

Requirements:

  • Minimum one (1) year experience as a consultant pharmacist in LTC/PAC facilities
  • BS Pharmacy or PharmD degree
  • Certified Geriatric Pharmacist (CGP) certification, preferred
  • Valid pharmacist license in the State of Maryland
  • Exceptional organizational skills and the ability to work independently and collaboratively
