Responsibilities
Key responsibilities of this role include:
- Coordinating patient care and ensuring timely and accurate documentation
- Collaborating with healthcare professionals to deliver exceptional patient outcomes
- Providing personalized care that addresses the unique needs and goals of each patient
- Ensuring accurate OASIS collection and medication profile management
- Attending weekly case conference and monthly case manager meetings
Qualifications
To be successful in this role, you will need:
- Current licensure as a Registered Nurse in the state of employment
- A minimum of one year of clinical experience
- Demonstrated knowledge and skill in current nursing practice
- A valid state driver's license and automobile liability insurance
