PRN Opportunity - Riverside Medical Group Staffing Support: Team members will have a minimum scheduling commitment of 8 hours per week or 24 hours per week. Shifts available in various locations with the ability to float to different Riverside Physician Practices. Overview

Works under the supervision of the office manager, RN or clinical lead, and the physicians. Responsible for assessing, planning, implementing, and evaluating total nursing care of assigned patients. The LPN may work in a variety of settings and roles within Riverside Medical Group providing hands-on clinical care to include manual vitals, medication review, injections, and blood draw. The LPN utilizes communication, critical thinking, problem-solving, and decision-making to create a care plan for each patient based on physiological, psychosocial, and cultural diversity. The LPN may be responsible for supervising and delegating non-professional personnel working directly with him/her to ensure optimum patient care. Must demonstrate competence in dealing with age-related needs of specific patient populations served in their assigned unit/dept. Works under the direction of leadership and a multi-disciplinary team to provide a comprehensive range of services or clinical care associated with and in accordance with standards of excellence established by Riverside Health System and facility goals and strategic plans. Float to multiple locations within the division providing support as needed.

Key Responsibilities
  • Accurately completes all required clinical documentation consistently per RHS standards. Maintains documentation for quality assurance to include but not limited to point of care testing, medication logs, and annual wellness visits. Works with providers to assure care gaps are routinely addressed with each patient. Documentation is accurate, legible, and completed same day. Knowledgeable and skillful in lab functions. Monitors OSHA and lab proficiency testing. Maintains clean, compliant, well-stocked work areas.
  • Provides patient education as needed. Teaches patients as necessary about the medications prescribed for them. Demonstrates positive patient care by explaining the importance of the completion of registration and patient medical history forms and how it relates to their care at our facility. Provides patient follow-up with a sense of urgency regarding lab calls, patient phone calls, prescription fills, and refills, referrals, and appointment scheduling.
  • Demonstrates the ability to build trusting collaborative relationships with a multi-disciplinary team to support patient care needs. Actively participates in multi-disciplinary team meetings as assigned.
  • Ensures proper reimbursement from insurance companies by performing pre-authorization for prescription drugs, procedures, and patient tests.
  • Answers telephone calls from patients, pharmacies, physicians, vendors, etc. by close of business. Communicates lab/pathology results and schedules additional tests per provider order.
Qualifications
  • Program Graduate, Licensed Practical Nurse (Required)
  • Bachelors Degree, Nursing (Preferred)
Experience
  • 1 year Nursing experience (Required)
  • 1 year Outpatient medical office experience (Preferred)
  • 1 year Experience using electronic medical record (EMR) (Preferred)
  • Less than 1 year Experience collecting and processing specimens in lab (Preferred)
Skills and Abilities
  • Knowledge of medications, their effects, drug interactions, medical procedures, and therapies, diagnosis, and diseases.
  • Ability to perform nursing assessments, patient interviews, and triage.
  • Ability to speak clearly and understandably.
  • Knowledge of medical terminology.
  • Good time management skills and self-starter.
  • Ability to maintain effective interpersonal relationships.
  • Proficiency in nursing equipment used in a clinic setting.
  • Skill in completing assignments accurately and with attention to detail.
  • Ability to work effectively independently or as part of a team with physicians and other staff.
  • Excellent communication skills both written and verbal with exceptional attention to detail.
  • Knowledge of patient rights and laws relative to those rights, such as HIPAA.
  • Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.
  • Ability to appropriately interact with patients, families, staff, and others.
  • Ability to flexibly respond to changing demands.
  • Ability to follow directions and apply proper policies, procedures, and guidelines.
  • Complete appropriate documentation in electronic medical record (EMR)/electronic health record (EHR).
