OBJECTIVE AND SCOPE:

The registered professional nurse (CAP RN 1) role serves as an introductory designation within the Clinical Advancement Program for Registered Nurses. The CAP RN 1 is accountable for delivering and coordinating clinically proficient care, which includes assessment, planning, intervention, and evaluation for a designated group of patients.

As a vital member of the Chronic Kidney Disease (CKD) healthcare team, the CAP RN 1 engages in decision-making, educational initiatives, leadership responsibilities, and quality enhancement activities that improve patient care outcomes and operational efficiency.

KEY DUTIES AND RESPONSIBILITIES:

All responsibilities are expected to be executed in alignment with Fresenius Kidney Care policies, procedures, nursing standards, and applicable regulations.

  • Executes essential functions under the supervision of the Clinical Manager (CM) and with guidance from the Educator, Preceptor, or a CAP RN 2, 3, 4, or 5 during the initial 6 months of employment.
  • Anticipated to fulfill requirements for advancement to the next CAP level within 24 months from the date of hire.
  • Conducts ongoing, systematic collection and analysis of patient data before, during, and after hemodialysis treatment for assigned patients, documenting findings in the patient medical record, and adjusting treatment plans as necessary while notifying relevant team members.
  • Identifies deviations in patient status and collaborates with appropriate healthcare team members for insights.
  • Evaluates daily patient care needs and collaborates with both direct and ancillary care staff as required.
  • Directs and ensures, in collaboration with care staff, the comprehensive and safe delivery of chronic hemodialysis therapy to assigned patients.
  • Administers medications as prescribed or according to approved protocols, documenting medical justification and effectiveness.
  • Ensures proper laboratory collection, processing, and shipping procedures are adhered to and reschedules any missed or inadequate laboratory collections.
  • Confirms that physician orders for assigned patients are accurately entered and acknowledged in the Medical Record.
  • Initiates or assists with emergency response protocols as necessary.
  • Delegates appropriate tasks to direct patient care staff, including but not limited to LVN/LPNs and Patient Care Technicians.
  • Monitors patient care staff for compliance with techniques and adherence to FKC policies and procedures.
  • Promotes infection control and ensures equipment and environmental safety.
  • Assesses, collaborates, and documents the educational needs of patients and families to provide initial and ongoing education.
  • Identifies expected outcomes, documents, and updates nursing assessments and care plans for assigned patients in collaboration with the Interdisciplinary Team.
  • Ensures patient awareness regarding transplant and treatment modality options.
  • Refers patients to Social Workers and Registered Dietitians as appropriate for tailored education and counseling.
  • Checks the availability of necessary emergency equipment and supplies as assigned.
  • Assists with medication tracking and inventory management as assigned.
  • Serves as a resource for the healthcare team and participates in training and orientation of new staff as assigned.
  • Engages in all required staff meetings as scheduled.
  • Supports and promotes the mission, vision, values, and customer service philosophy of FMCNA.
  • Performs additional duties as assigned by the supervisor.

PHYSICAL DEMANDS AND WORKING CONDITIONS:

  • The physical demands and work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this role. Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.
  • This position involves direct patient care that frequently requires heavy lifting and patient mobility assistance. Equipment aids and/or coworkers may provide support. The role necessitates prolonged periods of standing, and the employee must be capable of bending. Occasionally, the employee may need to move, with assistance, equipment weighing up to 200 lbs. and lift chemical and water solutions weighing up to 30 lbs. as high as 5 feet. The work environment is typical of a healthcare facility with controlled air temperature and moderate noise levels. Exposure to infectious and contagious diseases/materials may occur.
  • Daily responsibilities include desk work, computer usage, and interaction with patients, facility staff, and physicians. The position may require travel to training sites or other facilities.
  • The employee may be asked to perform essential functions of this position in other locations with similar physical demands and working conditions.

SUPERVISION:

  • Oversight of Patient Care Technicians/LPNs as a Team Leader or designated Nurse in charge is assigned after:
  • Successful completion of all FKC education and training requirements for new employees plus
  • A minimum of 9 months experience as an RN plus
  • Successful completion of 3 months experience in chronic hemodialysis.

EDUCATION:

  • Graduate of an accredited School of Nursing.
  • Current appropriate state licensure.
  • Must meet the practice requirements in the state of employment.

EXPERIENCE AND REQUIRED SKILLS:

  • Entry-level for RNs with less than 2 years of nephrology nursing experience within the last two years or for RNs with a BSN and less than 1 year of nephrology nursing experience within the last two years.
  • Minimum of 9 months experience as a Registered Nurse (preferred).
  • Chronic hemodialysis experience (preferred).
  • Successful completion of FKC Education and Training requirements for new employees.
  • Successful completion and maintenance of CPR BLS Certification.
  • Meet all conditions of employment including Ishihara's Color Blindness Test.
  • Successful completion of all annual required education/training/competencies.
  • Successful completion of ongoing training and education as assigned or required.
