MDS Coordinator

At JGS Lifecare, we are seeking a highly skilled and dedicated MDS Coordinator to join our team. As a key member of our healthcare team, you will play a vital role in ensuring the highest quality of care for our residents.

The MDS Coordinator will be responsible for conducting comprehensive assessments, developing and implementing care plans, and collaborating with our interdisciplinary team to ensure seamless care delivery. • Conduct thorough assessments of residents to identify their needs and develop individualized care plans
• Collaborate with our interdisciplinary team, including RNs and LPNs, to ensure comprehensive care delivery
• Develop and implement care plans that meet the unique needs of each resident
• Ensure compliance with state and federal regulations, as well as our organization's policies and procedures
• Document resident progress and communicate effectively with families and caregivers

• LPN or RN license is required
• Strong organizational and communication skills
• Ability to work effectively in a fast-paced environment
• Knowledge of healthcare regulations and policies
