We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team at Consulate Health Care. As an LPN, you will play a crucial role in providing high-quality nursing care to our residents, collaborating with our healthcare team to implement individualized treatment plans, and supervising and evaluating Certified Nursing Assistants (CNAs).
Key Responsibilities:
- Provide direct patient care, including administering medications, performing wound care, and assisting with daily living activities
- Collaborate with the healthcare team to develop and implement individualized care plans
- Supervise and evaluate CNAs, ensuring they are delivering care according to our high standards
- Conduct thorough evaluations of each resident's medical status upon admission and throughout their course of treatment
Requirements:
- Active license to practice as an LPN
- Experience in long-term care, skilled nursing, or rehabilitation facilities is preferred
