Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team as a Resource Nurse. In this role, you will provide nursing care to patients and families in their homes, hospitals, and other settings, and work collaboratively with our interdisciplinary team to ensure the best possible outcomes.

Key Responsibilities
  • Provide comprehensive assessments and develop individualized plans of care for patients and families.
  • Deliver high-quality nursing care, including medication management, wound care, and symptom management.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and coordination.
  • Communicate effectively with patients, families, and healthcare teams to ensure understanding and satisfaction.
  • Document patient information accurately and thoroughly in our electronic health record system.
Requirements
  • ADN or BSN degree from an accredited nursing program.
  • Current, unrestricted Registered Nursing License in the state of employment.
  • One year of nursing experience, preferably in hospice or end-of-life care.
  • Valid driver's license, auto insurance, and access to a reliable vehicle.
  • Ability to develop and maintain knowledge of hospice and end-of-life care processes.
  • Ability to maintain professional boundaries and confidentiality.
  • Basic Microsoft Office skills and intermediate electronic health records skills.
