Job Summary

We are seeking a skilled and compassionate Home Care Nurse Aide to join our team at Mid Michigan Health. As a Home Care Nurse Aide, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Responsibilities
  • Provide hands-on personal care and assist patients with daily activities such as bathing, dressing, and grooming.
  • Follow individual patient care plans to ensure accurate and timely completion of tasks.
  • Assist patients with ambulation, exercises, and other activities as needed.
  • Comply with infection prevention and control policies and procedures.
  • Develop and maintain positive relationships with patients and their families.
  • Document patient care and progress accurately and timely.
Requirements
  • High school diploma or equivalent required.
  • Valid driver's license and reliable transportation.
  • Ability to communicate effectively with patients, families, and healthcare team members.
  • Basic Life Support (BLS) certification required within 1 month of hire.
  • Vehicle insurance and fingerprinting required.
  • Ability to work independently and as part of a team.
Preferred Qualifications
  • Previous experience as a Home Care Nurse Aide or in a related field.
  • Knowledge of home health and hospice care principles and practices.
  • Ability to work with patients and families who are dealing with terminal illness, the dying process, and death.
Working Conditions

Exposure to stressful situations, including those involving public contact, trauma, grief, and death. Ability to wear personal protective equipment, including latex materials or appropriate substitutes. Must be able to move freely about the facility with or without an assisted device and perform the functions of the job as outlined in the job description.
