About the Role

We are seeking a skilled and compassionate Hospice Nurse Practitioner to join our team at Generations Hospice, a part of BrightSpring Health Services. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality, patient-centered care to individuals and their families.

Responsibilities
  • Provide comprehensive medical care, including evaluation, assessment, and management of patients with terminal illnesses, in compliance with state Nurse Practice Acts and under the supervision of a collaborating physician.
  • Conduct thorough history and physical examinations, evaluating patients' medical conditions and health histories.
  • Complete hospice face-to-face visit requirements to assess eligibility for certification and recertification.
  • Make recommendations to the hospice medical director regarding patient eligibility for certification and recertification.
  • Maintain accurate and detailed clinical notes, ensuring compliance with regulatory requirements.
  • Communicate effectively with physicians and participate in interdisciplinary group meetings to address patients' medical and emotional needs.
  • Ensure adherence to all state and federal laws and regulations governing hospice care.
Qualifications
  • Graduate from an accredited School of Nursing and accredited Nurse Practitioner program.
  • Current DEA license, if applicable to state regulations.
  • Current nursing license in the state of practice.
  • Current CPR certification.
  • Master's degree with a minimum of one year of NP experience.
  • One year of hospice or palliative care experience preferred.
  • Accreditation from an approved certifying body for advanced practice nursing, as required by the state of practice.
  • Certification in a specialist area, such as Hospice and Palliative Nursing (CHPN) or Geriatrics, is preferred.
  • Commitment to clinical and documentation excellence.
